I HEREBY AGREE TO WAIVE MY RIGHT TO A JURY TRIAL in any claim or lawsuit relating to my employment or potential employment with the Company – including those relating to my application for employment, any conduct occurring during my employment, and the termination of my employment (and including any statutory discrimination claims). I acknowledge and agree that this is a knowing and voluntary waiver of my right to a jury trial.
